Support foot canvas or Phone Bank over dial-up connection

If Canvassing by foot or by phone with a dial-up connection, permit the application to checkout a short list as a downloadable pdf or html form ready for printing and completion prepared in a way which facilitates its entry back into the database.

Provide a web form for loading canvas results from the printed form back into the database.

Obtain a form which will facilitate the collection of voterID and campaign support data, for households of voters. For each household represented on the form, it will include data related to the household and to the voters in that household, including:

Household Address and Phone Number known hhVoterID data for each voter in the household: first and last names, known voterID data, known additional phone numbers, and email, existing comments.

In addition the form will permit the collection and storage of data including:

contact status,
privacy (DNC) requests,
voter party preference,
voter email or additional contact data,
support status,
yardsign request,
request for followup call,
comments.
