The Green Party of the United States asked Campaign Foundations for a re-usuable Events Registration form to support organizing for their 2006 Annual Meeting in Tuscon AZ. We didn't have one to offer, but developed Organize::Registration in response. It took us four weeks and about 200 hours coding and testing to get from signing to initial launch. Here is what our forms look like on their site.

The application, soon to be released on CPAN under the GNU Public License is made available as a collection of perl modules, provides a web accessible event registration form, and is designed to use templates and a human readable configuration file to serve an event specifc form, build a database of registrants and their needs, generate an invoice and email confirmation and hand off to a credit card processor to complete the transaction.
